Upon registration, medical providers will be asked to provide the following information: tax identification number, company name, physician/hospital name job title, phone number, and email address. Delta Fund Services will not resell or share any of your personal information to commercial markets or private organizations. Personal information you provide us through email, or online questionnaires, will be used by Delta Fund Services only to respond to you.

Eligibility, benefits, and claims information is based on the data available. Final determination of payment is based on eligibility, fee allowances, medical necessity and contract exclusions and limitations in effect at the time service is provided.
